Form pehle Setup → Settings → Clinical → Custom Forms me banta hai; koi form na ho to "No custom forms created yet — go to Setup → Settings → Clinical → Custom Forms." dikhta hai. Patient ka naam, umar jaise token apne aap bhar jaate hain; signature box me naam pehle se patient ka bhara aata hai. E-sign wale form me naam khaali ho to "E-sign required — have the patient type their name." Laal * wale sawal abhi rokte nahi — khaali chhode to bhi save ho jaata hai, isliye check karke save karo. Bhara hua form edit nahi hota — galat ho to owner / admin 🗑 se hata kar dobara "Fill" karein; doosre login par 🗑 se kuch nahi hatta aur koi message bhi nahi aata. Koi bhi staff bhar sakta hai. Mobile: More → Records → "Forms" → "Custom" tab me bhare form ki list ("✓ Signed" / "Pending") aur "Open Forms".
Patient ke liye custom form (Form Builder wala) kaise bhare?
Patient chart → Sections → ⋯ More → Records → "Forms" → "Custom Forms" tab → form card par "Fill" → sawal bharo (signature wale form me patient ka naam type karo) → "Save submission" → "✓ Saved". Bhara form neeche "Filled forms (N)" me date aur "signed by" ke saath aata hai.
