Form khulte hi patient ke tokens (naam, UHID…) bhar jaate hain. Bhara form patient ke chart (⋯ More → Forms → Custom Forms → "Filled forms") aur Settings ke Filled forms list me "Via: admin" ke saath dikhta hai. Patient chart se seedha bharna ho to chart → ⋯ More → Forms → Custom Forms → form ka "Fill". Mobile: Filled forms → "Fill for a patient" → "Which form?" → patient search → jawab → "✓ Submit" ("via mobile"). Koi bhi staff login form bhar sakta hai.
Custom form kisi patient ke liye kaise bhare (Fill for a patient)?
Setup → Settings → Clinical → Custom Forms → "Filled forms" → "Fill for a patient" → Form chuno → Patient naam / phone / UHID se dhundo (kam se kam 2 akshar) → "Open form" → jawab bharo → "Save submission". Templates card ke "Fill" se bhi yahi khulta hai, form pehle se chuna hua.
