CAPA LIKHO
(1) Correction: is patient ke agle sitting par fresh consent (aage ke treatment ke liye). (2) Corrective: sab active files ka consent-completeness audit — IMS clinical record review / chart audit format use karo, findings CAPA register me. (3) Preventive: plan save hote hi khulne wala consent usi waqt print karke patient / guardian, witness aur doctor se sign karwao aur scan Docs me "Consent Form" naam se rakho — print-and-forget wala gap band.
APP ME DIKHAO
patient chart → ⋯ More → Forms → "Consent Forms" me us patient ke bane consent (date-time aur doctor ke naam ke saath), aur Docs → Documents me scan kiya sign wala kaagaz. App me screen par e-sign nahi hota aur consent ko "signed" mark karne ka button nahi hai — sign wala kaagaz hi saboot hai. MRD checklist register me 'Consent on file' chip bhi hai — discharge / completion par ye check hota hai.
