Form ek A4 page me fit hota hai: UHID, photo (li ho to), personal info, medical / dental history ke tick box, medicines, allergy, pain score, Consents (4.1–4.10 ke jawab) aur signature (patient signature / thumb; minor ho to guardian). Web par ye form hamesha Hindi + English (bilingual) chhapta hai — Printing Setup ki "Print Language" web registration form par abhi laagu nahi hoti; mobile print usi setting ko maanta hai (Setup → Settings → Billing → Printing Setup). Form par chhota QR hai — mobile app ke Patients → QR scan se patient seedha khulta hai. Sign karwa kar scan / photo lo aur chart ke "Docs" me upload karo. Patient card alag hai: chart ⋯ (More actions) → "Patient Card" / "🪪 Wallet Card (89×51mm)".
Patient ka registration form print ya dobara print kaise kare?
Save karte hi "Registration Form" khud khulta hai → "Print". Baad me: patient chart → sidebar "More" → "Forms" → "Registration" tab → "📄 Open print form" → "Print". Mobile: patient kholo → Profile tab ke "Quick Stats" me "Print Registration", ya upar 🖨 → "Registration slip".
