Tick pehle poore label se, warna synonym se lagta hai — haath se likha "HTN" bhi Hypertension par tick karega. Jo kisi box se na mile wo "अन्य / Other:" line par chhapta hai, kuch girta nahi. Current Medication aur Allergy ab apne alag column me, ek line par ek entry. Section 1 me ab Occupation, Referred By, Guardian (minor ke liye), Weight/Height aur Rate card bhi chhapte hain. Gender ab dono jagah mandatory hai ("Gender *"); purane bina-gender record par ☐ पु/M ☐ म/F ☐ अ/O box chhapte hain jo haath se tick kar sakte ho.
