Patient Connect → WhatsApp → "Lead Forms": "📋 Preset: Book Appointment" (naam, phone, email, service, date, notes) ya "+ New Form" — naam, URL slug, title, fields (text / phone / select / date…, Required) → "Save Form". Card par public link milta hai; aaye form "📊 Submissions" me New / Contacted / Converted / Lost. Beta — na dikhe to ORALZY support se poochho.
