Step 1: Title (Mr. / Mrs. / Ms. / Master / Baby), Name *, Phone * (10 digit) + "SMS DND", Gender *, Date of Birth ya Approx Age, Aadhaar, Father/Husband Name, Occupation, Guardian / Parent Name (18 se kam umar par zaroori), Guardian Relation, Weight (kg), Height (cm), Referred By, "Rate Card Plan" (sirf jab clinic ne rate plans banaye hon), "Govt / TPA scheme patient?" (sirf scheme wali clinic me). Step 2: Medical History, Dental History, Current Medicines, Allergies, Pain Scale, Habits (female par Pregnant? / Breastfeeding?). Step 3: Address (gaon / shehar ke naam suggest hote hain), Insurance Company / Insurance Type, Notes (mic button se bol kar bhi), recording consent, Patient Since, Groups, Tags, "🧾 Consultation Fee" box (fee on ho to), aur "📜 Consent Points (DPDP Act 2023)". Save ke baad Registration Form ka "Print" / "Close" aata hai — "Close" dabate hi naye patient ka chart khul jaata hai. Calendar ke "+ New PT" se aaye ho to appointment bhi khud ban jaati hai. Mobile: Register patient screen par sab ek hi scroll me, neeche "Register Patient" button.
Naya patient register kaise kare?
Patient Care → Patients → Register New Patient (ya upar top bar ka hara "New Patient" button) → 3 step: "Basic Info" (Name, Phone, Gender, DOB/Age, photo) → "Next: Medical History →" (history, allergy, pain) → "Next: Contact →" (address, fee, Consent Points) → "Save Patient ✓". Save hote hi UHID banta hai aur "Registration Form" print ke liye khul jaata hai.
