Setup → Settings → Clinical → Treatment Types → "Edit" → "Where / billed per:" dropdown: "Tooth (one row per tooth)", "Sextant — price is PER sextant", "Quadrant — price is PER quadrant", "Arch — price is PER arch", "Full mouth — one price", "Soft tissue — one price", "Any / not tooth-specific — one price" → "Save".
