Score patient ke record me save hota hai aur printed registration form ke Section 2 me "दर्द का स्तर (0–10) / Pain score (0–10)" line par 11 box chhapte hain — chuna hua tick, kuch na chuna ho to sab khaali (haath se tick kar sakte ho). Ye sirf pehli visit ka score hai; har visit ka pain before/after case sheet ke Clinical Notes me alag hai.
